If you currently have filed or have a vision claim to submit, please send it directly to me. If you have sent in a form already and have not been reimbursed, please re send it to me and I will look into all claims that have been submitted.
The system is not processing these claims properly and until we have it fixed I will be handling these claims again.
The procedure is:
Fill out a claim form and send me a SCANNED copy of it along with your receipt for services.
